Documentation Coding Guide

This 148 page book is distributed to attendees at seminars on documentation for hospital billing. The book serves as a basic reference for medial services documentation, coding, and billing.
The original version was created in 1999, and I took this opportunity to make several improvements, leading to a format better suited to the way the manual is used. The first change was in the size of the manual from 8 1/2" X 11" to a much handier and easily stored 5" X 8". The stock chosen for this printing was a heavier, sturdier paper to survive a lot of handling. The binding was changed from perfect bind to wire ring binding, allowing the person using it to leave it open for reference without having to hold the pages down. The design of the interior was overhauled for more consistency and easier reading. The instructor was very happy with the results and agreed the changes made the book better suited for its audience.
